Have you changed address?

If you have recently moved, it is very important that you tell us. This is because from time to time we may need to contact you. There is also a danger the Primary Care Trust may ask us to return your medical records if they have tried to contact you unsuccessfully and this may leave you in the unenviable position of not being registered with a GP at all.

If you have moved, we need confirmation in writing, please remember to make it clear if other family members have moved with you or not. In the case of families splitting or children moving between parents, we may require confirmation from your ex-partner or spouse.

Practice Boundary

Like all practices, we can only see patients within a specific area. The main reason for this is that we cannot commit to taking care of people who live too far from the surgery, because if you are very ill and we have to come out and visit you, the time taken to do this can cause unacceptable waiting times or even cancelled appointments for our other patients.
